File of Life Card
The file of life is a mini-medical history that provides emergency personnel with helpful information during critical and/or life-threatening situations. The card, for each household member, lists the patient's name, emergency medical contacts, medical history, and medication usage.
Managing the Information
The information is kept in a red, plastic pocket file with a magnet that is held to the refrigerator/ freezer door. Its visibility and readability ensure that emergency personnel can access the card quickly when responding to a call, especially if the patient is confused, frightened or unable to offer assistance regarding needed information.
Update this form whenever you have a change of medication or medical history. Keep a copy of this form in your File of Life magnetic packet, which should be placed on your refrigerator. A copy of this form also should be kept in your wallet or purse in case of emergency.
