Mobile Integrated Health

Huntley Fire Protection District is proud to provide a Mobile Integrated Healthcare (MIH) Program. This program is designed to support and assist HFPD's EMS system in caring for patients on a non-emergent basis within the community.
What is Mobile Integrated Health (MIH)?

MIH, also known as a Community Paramedic Program, is the provision of healthcare using patient-centered, mobile resources in the out-of-hospital environment. Paramedics from Huntley Fire Protection District will take care of residents in their communities who are the most at-risk and in need of additional help. Paramedics will visit the patients in their home to assess for any safety and medical needs. This service will ultimately reduce unnecessary transports and help provide a collaborative system of care and education through community risk reduction.

  2. Fire Safety

  3. Home Trip/Fall Hazard Prevention

  4. Premise Alert Program/File For Life Program

  5. Blood Pressure/Blood Sugar Assessments

  6. Medication Reconciliation

  7. 911 Follow-Up Concerns

  8. Referrals for:

  • Senior Services

  • Meals on Wheels

  • Home Care and Companion Aids

  • Transportation Services
